URL与ESWL治疗老年输尿管结石的疗效分析

目的]探讨输尿管镜下气压弹道碎石(URL)和体外冲击波碎石(ESWL)治疗老年输尿管结石的疗效。方法]回顾性分析2007年1月～2010年12月在某院分别采用URL与ESWL治疗的输尿管结石老年患者临床资料。结果]URL组总成功率和中下段结石成功率明显高于ESWL组,差异有统计学意义(均P﹤0.01);上段结石成功率低于ESWL组,差异有统计学意义(P﹤0.05)。URL组发热和肾绞痛均明显少于ESWL组,差异有统计学意义(均P﹤0.01);恶心呕吐、肉眼血尿和输尿管穿孔差异均无统计学意义(均P﹥0.05)。结论URL在微创性上更有优势,ESWL应作为老年输尿管上段结石的首选,URL应作为老年输尿管中下段结石的首选。

ANALYSIS OF THE CURATIVE EFFECT OF URL AND ESWL ON URETERAL CALCULI TREAT OF THE OLD PATIENTS

Objective]To investigate the therapeutic effect of Ureteroscopic Lithotripsy(URL)and Extracorporeal Shock Wave Lithotripsy(ESWL)on the ureteral calculi in the old patients.Methods]A retrospective analysis was carried out for the therapeutic effect of URL and ESWL treatment on ureteral calculi of elderly patients from January 2007 years to December 2010.Results]Total success rate and the success rate of lower ureter in URL group were higher than those of ESWL group(P﹤0.01);The success rate of upper ureter in URL group was lower than that of ESWL group(P﹤0.05).Fever and renal colic were less in URL group than those in ESWL group(P﹤0.01);Vomiting,gross hematuria and ureteral perforation had no significant difference between the 2 groups(P﹥0.05).Conclusion]Minimally invasive URL treatment has more advantages.ESWL should be the firest choice with upper ureteral calculi,and URL should be the first choice for lower ureteral calcuili for the old patients.
