慢性输卵管炎致不孕75例的临床治疗

目的采用中药口服并结合输卵管通液、经期抗炎治疗观察输卵管炎性阻塞性不孕的临床疗效。方法按照随机数字法将75例输卵管炎性阻塞性不孕症患者随机分为观察组(n=35)和对照组(n=40),其中观察组采用中药口服并结合输卵管通液、经期抗炎治疗,对照组仅进行输卵管通液、经期抗炎治疗,比较两组患者的临床疗效。结果观察组输卵管炎性阻塞性不孕症患者中痊愈12例,显效11例,有效8例,总有效率达88.57%;对照组输卵管炎性阻塞性不孕症患者中痊愈6例,显效6例,有效10例,总有效率达55.00%;经统计分析发现观察组的总有效率明显高于对照组,且差异具有统计学意义(χ2=10.15,P﹤0.05)。观察组治疗后综合积分较治疗前明显下降,且差异具有统计学意义(t=2.63,P﹤0.05);而对照组治疗后综合积分则与治疗前无明显差异(t=1.16,P﹥0.05)。本组75例患者中共有150条输卵管发生阻塞,其中观察组共有70条,对照组80条;经过积极的治疗,其中观察组治愈28例,有效31例,总有效率达84.29%;对照组治愈15例,有效40例,总有效率为68.75%,经统计分析,观察组的治愈率和总有效率均明显高于对照组,且差异具有统计学意义(χ2=8.24、4.94,P﹤0.05)。结论传统的输卵管通液术对输卵管复通有一定作用,但治疗有效率低,以化瘀通络汤口服以及输卵管通液相结合的中西医结合方法可以有效扬长避短,提高了治疗有效率和输卵管通畅率,具有临床应用和推广价值。

Clinical study of 75 cases of infertility induced by chronic tubal inflammation

OBJECTIVE To investigate the clinical curative effect of oral traditional Chinese medicine combined with hydrotubation,anti-inflammatory treatment during menstrual period in tubal inflammatory obstructive infertility.METHODS According to the random number method,75 cases of infertility patients with obstructive salpingitis were randomly divided into the observation group(n = 35)and control group(n = 40).In the observation group,oral Chinese medicine combined with hydrotubation,anti-inflammatory treatment during menstruation were used,the control group was only given with anti-inflammatory therapy in menstrual period.Then compared the clinical efficacy in two groups.RESULTS 12 cases were cured in the observation group,11 cases obviously effective,8 cases effective,the total effective rate was 88.57%;15 cases were cured in the control group,40 cases effective,the total effective rate was 55.00%;by statistical analysis,the total efficiency in the observation group was significantly higher than that of the control group,and the difference was significant(χ2 = 10.15,P﹤0.05).The score decreased significantly in the observation group after treatment than before treatment,and the difference was significant(t = 2.63,P﹤0.05),while in the control group,the score had no significant difference(t = 1.16,P﹥0.05).150 tubal obstruction occurred in 75 patients,70 uterine tubes in the observation group,80 in control group;after active treatment,28 cases were cured in the observation group,effective in 31 cases,the total effective rate was 84.29%;15 cases were cured in the control group,effective in 40 cases,the total effective rate was 68.75%.Through statistical analysis,the cure rate and total effective rate in the observation group were higher than those in the control group,and the difference was significant(χ2 = 8.24,4.94,P﹤0.05).CONCLUSION The traditional tubal surgery has a certain effect,but the treatment is with low efficiency.The method of integrated tcm-wm that oral traditional Chinese medicine and hydrotubation can effectively foster strengths and circumvent weaknesses,improve the treatment efficiency and tubal patency rate,which has clinical value for application and popularization.
