纤支镜灌洗治疗60例重症肺部感染的临床分析

[目的]探讨重症肺部感染采用纤维支气管镜(纤支镜)肺泡灌洗及注入药物治疗的临床疗效。[方法]选取2008～2010年确诊为重症肺部感染的60例患者,随机分为灌洗组和对照组各30例,灌洗组在抗生素及综合治疗的基础上给予经纤支镜肺泡灌洗,对照组患者给予抗生素及综合治疗,每周2次,连续2周后对两组治疗效果进行临床观察和统计学分析。[结果]灌洗组和对照组致病菌清除率分别是81.8%和42.1%,两组比较差异有统计学意义(P﹤0.05);灌洗组的治愈率(70.0%)和有效率(20.0%)与对照组的治愈率(30.0%)和有效率(36.7%)相比,差异均有统计学意义(P﹤0.05)。[结论纤支镜肺泡灌洗治疗重症肺部感染的疗效显著且安全,值得临床推广应用。

CLINICAL ANALYSIS OF BRONCHOFIBERSCOPIC LAVAGE ON 60 PATIENTS WITH SEVERE PULMONARY INFECTION

[Objective]To investigate the clinical effect of bronchofiberscopic lavage and drug injection by fibrobronchoscopy in the treatment of severe pulmonary infection.[Methods]60 severe pulmonary infection patients were selected from our hospital between 2008-2010,whom were divided into the lavage group(30 cases)and control group(30 cases).Antibiotics and general therapy were given in the control group,on the basis of that,then bronchofiberscopic lavage and drug injection were given to the lavage group,twice a week,for two weeks.Then conducted clinical observation and statistical analysis in the two groups after two weeks.[Results]The clearance rate of bacteria in the lavage group was 81.8%,which was much higher than 42.1% in the control group(P﹤0.05).The cure rate and efficiency rate in lavage group were 70.0% and 20.0%,compared 30.0% and 36.7% in the control group,there were significant differences between two groups(P﹤0.05).[Conclusion]Bronchofiberscopic lavage is very effective and safe in treating refractory pulmonary infection and hence it is worthy of being recommended in clinical practice.
