延迟脐带结扎用于早产儿低血糖防治的临床分析

[目的]了解早产儿低血糖发生的情况,探讨延迟脐带结扎对早产儿低血糖的防治效果,总结延迟脐带结扎用于临床防治早产儿低血糖的经验。[方法]将60例早产低血糖患儿随机分为两组,对照组30例,在婴儿出生早期通过静脉补充葡萄糖进行治疗,观察组30例,在对照组早期治疗的基础上,于婴儿出生时对其脐带结扎时间延迟30s～2min,然后观察两组患儿5h,1d,2d,3d的血糖和1,2,4,8周的Hb的动态变化情况并进行比较分析,同时分析导致早产儿低血糖的原因。[结果]两组早产儿出生后的血糖动态变化情况相比差异有统计学意义,P﹤0.05;两组早产儿出生后的Hb动态变化情况相比差异有统计学意义,P﹤0.01。经过回归分析发现,胎龄和体重是导致早产儿低血糖主要因素。[结论]在临床上,延迟脐带结扎对防止早产儿低血糖的效果非常显著,可以明显提高葡萄糖输注治疗的效果。胎龄和体重是影响早产儿低血糖的高危因素,这为临床进行早产儿低血糖防治提供了指征经验。

CLINICAL ANALYSIS OF DELAYED UMBILICAL CORD LIGATION FOR PREVENTION AND TREATMENT OF PREMATURE HYPOGLYCEMIA

[Objective]To understand the occurrence of premature hypoglycemia,delayed umbilical cord ligation on premature hypoglycemia prevention effect,summing the delayed umbilical cord ligation for clinical prevention and cure of premature hypoglycemia experience.[Methods]60 cases of premature hypoglycemia in children were randomly divided into two groups,the control group(30 cases)were performed intravenous supplementation of glucose treatment in new born infants,and the umbilical cord ligation time of observation group(30 cases)in babies delayed 30 seconds to 2 minutes,then we observed two group in 5 hours,1 days,2 days and 3 days of glucose and 1,2,4 and 8 weeks in the dynamic changes of Hb and analyzed and compared,and the cause of hypoglycemia in premature infants were analyzed.[Results]The two groups of premature birth after glucose changes compared with significant statistical difference,P﹤0.05;The two groups of premature birth after Hb changes compared with significant statistical difference,P﹤0.01.After the regression analysis,gestational age and weight are main factors lead to premature hypoglycemia.[Conclusion]In clinical,delayed umbilical cord ligation significant prevent premature hypoglycemia effect,which can significantly improve the therapeutic effect of glucose infusion.Gestational age and weight were the effect of premature hypoglycemia risk factors,the clinical prevention and treatment of hypoglycemia in premature infants provided indications of experience.
