彩色多普勒超声诊断急性阑尾炎

目的：探讨彩色多普勒超声诊断急性阑尾炎的价值。方法：应用彩色多普勒超声加压法探测163例临床拟诊急性阑尾炎者。结果：75例超声提示急性阑尾炎并术后病理证实，其中51例显示急性阑尾炎的直接征象，24例则仅见下列间接征象；（1）17例见阑尾区局限性肠管激惹征；92）5例见阑尾区混合性包块；（3）2例腹部压痛明显处见局限性少量积液伴周围脏器炎性反应，7例急性阑尾炎患者超声未见异常征象。结论；结合超声直接征象与间接征象可明显提高诊断急性阑尾炎的准确性。

Diagnosis of acute appendicitis with color doppler ultra sonography

Objective: To investigate the value of sonography in the diagnosis of acute appendicitis (AA). Methods: 163 cases suspected clinically with AA were examined with sonography. Results: Among the 163 cases,75 were diagnosed both by preoperative sonography and postoperative pathology,and 7 without positive sonographic findings were diagnosed during the operation. The direct and the indirect sonographic signs of AA were seen in 51 and 24 cases respectively. The inderect sonographic signs were as follows: excitatory enterokinesia limited around appendix (17/24); mixed mass in appendix area (5/24); limited intra abdominal exudate complicating the inflammation of the organs around it, which was at the position of abdominal tenderness (2/24). Conclusion: Ultrasonography could be conducive to the diagnodis of AA.
