椎管内神经鞘瘤的临床诊断和手术治疗

目的总结椎管内神经鞘瘤的临床诊断和手术治疗经验。方法回顾性分析38例椎管内神经鞘瘤的临床表现、影像学资料、手术方式以及手术前后患者脊髓或神经功能改善情况。结果MRI检查显示对病变性质和部位诊断有重要意义；38例患者肿瘤均完整切除、病理证实，所有患者术后神经功能均有明显恢复。结论影像学检查对神经鞘瘤有较高的诊断率，最终确诊依赖于病理诊断；椎管内神经鞘瘤治愈率高，临床效果好；行手术全部切除是可行的，熟练的手术技巧及肿瘤全切是取得良好效果的关键。

Clinical diagnosis and surgical treatment of intraspinal schwannoma

Objective To summarize the diagnosis and surgical treatment of intraspinal Schwannomas. Method A retrospective study from our department in December 2006 -2008 was practised in 38 cases of intraspinal schwannoma of clinical manifestations, imaging data, surgical approach, as well as before and after surgery in patients with spinal cord or nerve function to improve the situation. Result MRI examination was important to reveal the nature and location of the lesion; tumors were completely resected in 38 cases , pathologically confirmed, and all patients had obviously the restoration of neurological function. Conclusion Imaging examination of schwannoma has a high diagnostic rate,but the final diagnosis relies on pathological diagnosis. Intraspinal schwannoma has high cure rate and good clinical results. Surgical resection is feasible, and skilled surgical techniques and total resection of tumor are the key to good results.
