口服糖耐量试验中血糖及相关激素水平观察

摘    要:目的:研究口服糖耐量试验(OGTT)过程中胰岛素、胰高血糖素、生长抑素、瘦素对血糖的影响及相互关系。方法:选择我院内分泌门诊初诊者38人,在作OGTT的同时测定胰岛素、胰高血糖素、生长抑素和瘦素,分析这些激素水平与血糖浓度变化的关系。以及这些激素相互之间的关系。结果:在38人中,OGTT显示9个糖耐量正常,29例为糖尿病或糖耐量异常,与糖耐量正常组比较,糖耐量异常组患者的血糖值显著性增高,胰岛素峰值延后,两组的胰高血糖素均有分泌峰,但激素水平组间无差异,生长抑素和瘦素均无明显的峰值,组间也无差别。结论:在OGTT过程中,胰岛素和胰高血糖素随血糖的变化而变化,在血糖调节中起主要作用。而生长抑素与瘦素对维持血糖的稳定作用较小。

Abstract:Objective To study the effects of insulin, glucagon , somatostatin and leptin in maintaining plasma glucose level during oral glucose tolerance test ( OGTT) and the relationship among these hormones. Methods Thirty - eight subjects from the endocrine clinic of our hospital were selected. During OGTT, plasma glucose , insulin, glucagon, somatostatin and leptin of these subjects were measured simultaneously, and the relationship between these hormones and plasma glucose was analyzed. Results Nine subjects had normal glucose tolerance and 29 subjects were diabetic or impaired in glucose tolerance. Compared with the normal glucose tolerance group, those in the abnormal glucose tolerance group had significantly higher plasma glucose and delayed insulin peak values. The glucagon levels of both groups increased initially and then decreased. No significant difference was found between the two groups. Somatostatin and leptin levels did not change significantly during the period and no difference existed between the two groups. Conclusion During OGTT, insulin and glucagon levels change with the plasma glucose concentration, and they may play a key role in maintaining glucose concentration. Somatostatin and leptin have fewer effects in glucose regulation.
