早产儿静脉补糖时血糖及胰岛素的观察

36例早产儿随机分为两组,小剂量A组20例,高剂量B组16例,日龄2～10天,平均5.56±3.03天。静脉输糖速率分别为8mg·kg~(-1)/min及12mg·kg~(-1)/min,观察空腹时、补糖3h及停止补糖后3h的血糖及胰岛素水平。结果表明高剂量B组婴儿血糖值显著高于A组,高血糖发生率高达100％。两组各时项血清胰岛素值比较无显著性差异。提示早产儿胰岛素β细胞对较高剂量的葡萄糖输入缺乏合适的应答反应,为避免高血糖,早产儿静脉补糖时需控制补糖剂量。

Observations on blood glucose and insulin values in premature neonates given dextrose infusion

Thirty-six premature neonates were randomly devided into two groups, group A (n= 20) and group B( n =16), they were given glucose infusion at a rate of 8mg ?kg-1/min and 12mg ?kg-1/min respectively. Blood glucose and insulin values were observed during fasting, during three hours glucose infusion and at three hours after glucose infusion. The results showed the mean blood glucose value in group A were normal, while in group B, they were all in hyperglycemic states during 30 to 180min glucose infusion. But there were no significant difference of serum insulin values be-tween two groups . It is concluded that intravenous glucose infusion rate of 8mg ?kg-1/min is appro-preate for premature infants and the beta cells didn't response to a higher dose of glucose infusion in prematare infants. It is worth to further study why the insulin secretion of the beta cells in these in-fants responsed similarly to the different rates of glucose infusion.
