原发性阑尾黏液腺癌的诊断与治疗(复习文献34例分析)

目的 探讨原发性阑尾黏液腺癌的诊断和治疗。方法 对1980～2005年国内文献报告的34例和我院发现的1例患者的临床资料进行回顾性分析。结果 该35例患者的主要临床表现为右下腹疼痛，右下腹近腹股沟区包块及腹痛、腹泻伴便血；术前诊断为急性阑尾炎11例，慢性阑尾炎5例，阑尾周围脓肿7例，回盲部肿瘤5例。结论原发性阑尾黏液腺癌在临床上非常罕见，术前无明确诊断者，诊断强调术中仔细鉴别，B超、胃肠钡餐X线检查，CT或纤维结肠镜检查有助于诊断，确诊有赖于病理，治疗以手术为主，放化疗为辅。

Diagnosis and Treatment of 35 Cases of Primary Adennocarcinoma of the Vermiform Appendix

Objective To discuss the diagnosis and treatment of 35 cases of primary adennocarcinoma of the vermiform appendix.Methods To review our experience of 34 patients reported by national magzations from 1980 to 2005 and one ease undergoing treatment of primary adennocarcima of the vermiform sppendix in our hospital. Results The main clinical finding is pain of the right lower quadrant.In 35 cases,before operation, 11 were acute appendicitis and 5 were chronic appendicitis. 7 were appendiceal abscess and 5 were ileocecal tumor. Conclusion Primary adennocarcinoma of the vermiform appendix in clinical very rarely,all cases had no diagnosis before that operation, emphasize careful discriminate, B- ultrasound barium enema and CT or fiberoptic colonoscope examination is beneficial to the diagnosis, and diagnosis is depended on in pathology, the treatment regard surgical operation as principle,chemoradiotherapy is assist.
