强直性脊柱炎的护理干预

目的 探讨护理干预对强制性脊柱炎(ankylosing spondylitis,AS)患者预后的影响,以进一步加强此病的护理,掌握更多护理技巧.方法 对2009年1月-2010年12月收住新疆医科大学附属中医医院脊柱二科的58例AS患者进行有针对性的心理护理、体位护理、功能康复护理、服药护理及饮食护理,观察其预后.结果 经过医护人员的精心护理,58例患者腰背痛、关节痛减轻,疼痛评分由入院的8～10分下降到2～5分,晨僵时间由治疗前的(75.0±20.1)min下降到(43.7±19.3)min,治疗后晨僵时间较治疗前明显减少,差异有统计学意义(P<0.05).治疗后指地距为(18.2±8.3)cm,C反应蛋白(CRP)为(15.3±4.1)mg/L,较入院时的(30.1±11.3)min、(26.7±7.3)mg/L有所改善,差异有统计学意义(P<0.05).治疗中少数患者出现胃肠道反应,给予对症治疗后症状缓解.结论 精心的护理能有效地提高强直性脊柱炎的疗效,系统的护理和康复训练,可减少关节畸形的发生,提高患者的生活质量,最大限度地降低病残率.

Nursing intervention of ankylosing spondylitis

Objective To explore the nursing intervention on prognosis of the mandatory spondylitis(AS),summarize the nursing management experience to further strengthen disease care,and master more care and skill.Methods From January 2009-December 2010,58 patients were admitted to our hospital with AS,who were targeted psychological care,postural care,functional rehabilitation care,medication and diet nursing care,observed the outcome of the patients.Results After medical intensive care,the sore of low back pain,joint pain reduced pain of 58 patients down to 2 to 5.The duration of morning stiffness decreased from(75.0±20.1) min to(43.7±19.3) min,time of morning stiffness after treatment was significantly reduced than before treatment,the difference was statistically significant(P<0.05).The distance of finger and floor after treatment was(18.2±8.3) cm,C-reactive protein(CRP) was(15.3±4.1)mg/L compared with the time of admission respectively(30.1±11.3) min,(26.7±7.3) mg/L].There were few gastrointestinal reactions,they were given symptomatic relief of symptoms after treatment.Conclus-ion Careful care can effectively improve the efficacy of ankylosing spondylitis,the system of care and Rehabilitation training can reduce the incidence of joint deformity and improve quality of life of patients,maximum drop the morbidity.
