老年慢性阻塞性肺气肿合并气胸患者的临床诊疗分析

目的:通过分析老年慢性阻塞性肺气肿合并气胸的临床特点,探讨有效的临床诊疗方法。方法:对我院2008—2011年间收治的56例老年慢性阻塞性肺气肿合并自发性气胸患者的临床资料进行回顾性分析。结果:本组56例患者总确诊率为78.6%,气胸症状典型组患者确诊率(86.4%)高于不典型组,两组间差异性比较未见统计学意义(P>0.05)。46例患者在治疗2周后痊愈出院,总治愈率达82.1%,高龄患者(71～84岁)治愈率低于对照组,不同年龄组间治愈率差异无统计学意义(P>0.05),7例患者临床症状未有显著性变化,3例高龄患者因感染和并发症等原因导致死亡。结论:及时对老年慢性肺气肿合并气胸患者确诊,并根据不同的临床症状采取积极有效的对症治疗。同时加强对基础病的观察和治疗,尤其是高龄患者,应对其病史和病情有详细的了解,加强护理,降低病死率。

Analysis of Diagnosis and Treatment in Elderly Chronic Obstructive Pulmonary Patients with Pneumothorax

Objective:By analysing clinical features of elderly patients with chronic obstructive emphysema with pneumothorax,to explore the effective method for clinical diagnosis and treatment.Methods:The data of 56 cases which were elderly chronic obstructive pulmonary patients with pneumothorax in our hospital from 2008 to 2011 years were retrospectively analyzed.Results:56 cases of patients with total positive rate was 78.6%,pneumothorax symptoms typical groups of patients diagnosed rate(86.4%)was higher than those of the typical group,the difference between the two groups was no statistical significance(P>0.05).46 consecutive patients were cured after 2 weeks,the cure rate was amounted to 82.1%,elderly patients(71 to 84 years old)cure rate was lower than that of the control group,the cure rate differences between different age groups was no statistical significance(P>0.05),7 cases of the clinical symptoms of patients was no significant changes,in 3 cases of elderly patients died because of infection and complications.Conclusion:Diagnose elderly patients with chronic obstructive emphysema with pneumothorax timely,and according to the different clinical symptoms to take positive and effective symptomatic treatment.At the same time,strengthen observation and treatment of the basis of disease,especially in elderly patients,must have a detailed understanding of their history and condition,strengthen nursing,reduce the mortality rate.
