剖宫产术后阴道分娩的探讨

目的探讨剖宫产后再次妊娠经阴道分娩的安全性和可行性。方法取剖宫产术后再次妊娠的病例324例为研究对象,根据具体情况进行再次择期剖宫产或经阴试产,试产失败者改行剖宫产。对不同分娩方式的产程、产后出血、子宫复旧、新生儿Apgar评分等情况进行回顾性分析。结果剖宫产术后经阴试产151例,阴道分娩成功98例(有阴道试产经历者60例,无阴道试产经历者38例),有阴道试产经历者产程更快,阴道分娩成功率更高(P<0.01)。不同分娩方式新生儿Apgar评分比较差异无显著性(P>0.05)。剖宫产后阴道分娩与再次剖宫产比较,产后出血量低,子宫复旧时间短,差异有显著性(P<0.01)。结论只要选择恰当的病例,严密的产程管理,剖宫产术后阴道分娩是安全有效的,有阴道试产经历者更宜试产。
