感染性休克时血流动力学的改变与预后的关系探讨

摘    要:目的 分析感染性休克时血流动力学的改变与预后的关系.方法 回顾性分析医院2008年11月-2010年10月感染性休克患者71例的临床资料,按照患者预后情况分为生存组37例和死亡组34例,比较分析两组患者感染性休克时血流动力学的改变情况.结果 两组患者的一般资料间差异无统计学意义;生存组患者的心脏指数(CI)和左心、右心每搏功( LCSW、RCSW)分别为(4.8±1.1)L/(min· m2)、(42.5±4.1)g/m、(11.6±0.9)g/m,均明显优于死亡组的(6.9±1.5)L/(min· m2)、(60.3±4.7)g/m、(19.2±1.2)g/m,组间差异均有统计学意义(P<0.05).结论 感染性休克时血流动力学的改变影响患者的预后,血流动力学指标可以作为判断感染性休克患者预后的观察指标.

Abstract:OBJECTIVE To investigate the relationship between the changes of hemodynamics and prognosis as septic shock happened.METHODS The clinical data of 71 cases with septic shock treated in our hospital from Nov.2008 to Oct.2010 were retrospectively analyzed and divided into the survival group with 37 cases and the death group with 34 cases in accordance with the prognosis of patients.The changes of hemodynamics with septic shock of the two groups were compared and analyzed.RESULTS The general information between the two groups was not significantly different(P>0.05).The cardiac index(CI) for the patients of the survival group and the stroke power(LCSW,RCSW)for the left ventricular and the right ventricular were(4.8±1.1)L/(min.m2),(42.5±4.1) g·m/m2,(11.6±0.9)g·m/m2,respectively,which were significantly better than the death group(6.9±1.5)L/(min.m2),(60.3±4.7)g·m/m2,(19.2±1.2)g·m/m2.The differences between two groups were significantly different(P<0.05).CONCLUSION The changes of hemodynamics of the patients with septic shock affect the prognosis.The index of hemodynamics can be used as the observation index to predict the prognosis of patients with septic shock.
