开胸术后中心静脉导管相关性感染危险因素的分析与防治

目的 探讨开胸手术后中心静脉导管相关性感染(CVC-RI)发生的原因及防治方法.方法 对392例开胸手术后患者中心静脉导管感染的发生情况进行回顾性分析.结果 392例患者中共103例发生医院感染,感染率为26.3％；发生开胸术后中心静脉导管相关性感染危险因素包括:年龄、导管留置时间、部位、技术质量、机体免疫力、全胃肠外营养(TPN)治疗时间、入住ICU.结论 开胸术后患者CVC-RI的发生与上述危险因素有关,医院应建立CVC-RI的预防指引,建立操作资格准入制度,健全质量监管体系,加强导管的护理,以降低CVC-RI发生率,提高中心静脉置管输液护理的安全性.

Investigation and preventive measures on risk factors of central venous catheter-related infection after thoracotomy

OBJECTIVE To explore the causes and treatment of central venous catheter-related infection(CVC-RI) after thoracotomy.METHODS A retrospective analysis was carried out with a total of 392 patients with central venous catheter-related infection.RESULTS Among the 392 patients,there were 103 cases of nosocomial infection.The incidence was 26.3%.The risk factors of CVC-RI after thoracotomy included age,catheter detaining time,retention site of catheter,quality of catheterization,immunity,duration of total parenteral nutrition(TPN) and experience of ICU stay.CONCLUSION The above factors are correlated with the incidence of postoperative CVC-RI.To reduce CVC-RI and ensure the nursing security of central venous catheterization,it is necessary to establish a prevention guidance for CVC-RI,build an access system of operative qualification,strengthen the catheter nursing and improve the system quality of supervision and management.
