膜式液基薄层细胞制片技术与阴道镜联合应用诊断宫颈上皮内瘤样病变的价值评价

目的:评价膜式液基薄层细胞制片技术与阴道镜联合应用诊断宫颈上皮内瘤样病变的准确性。方法:选择妇科门诊妇女2400例进行宫颈病变筛查,随机平均分为两组,实验组1200例做宫颈膜式液基薄层细胞制片技术,对照组1200例做宫颈巴氏涂片法检查,比较两组宫颈异常细胞的检出率,并对细胞学检查阳性患者(ASCUS及以上)进行阴道镜下活检对照,观察阳性符合率。结果:(1)膜式液基薄层细胞制片技术和巴氏涂片法对宫颈异常细胞的检出率分别为8.58%和3.75%,两组结果差异有统计学意义(P<0.01)。(2)膜式液基薄层细胞制片技术与阴道镜下活检的阳性符合率为71.84%,巴氏涂片法为35.56%,两组结果差异有统计学意义(P<0.01)。结论:膜式液基薄层细胞制片技术检查结合阴道镜下定位活检可提高宫颈上皮内瘤样病变诊断的准确性。

Evaluation of application of Thih-Prep cytolosy test associated with colposcope to diagnose cervical intraepithelial neoplasia

Objective To evaluate the accuracy of application of Thih-Prep cytolosy test associated with colposcope to diagnose cervical intraepithelial neoplasia.Methods Two thousands four hundred women from the gynecological outpatients were chosen to screen cervical lesions,1200 of which received the Thih-Prep cytolosy test(experimental group),the other 1200 received the cervical examination of conventional smear(control group). The relevance ratio of cervical abnormal cells were compared between two groups and compared with the biopsy under colposcope for the patients with positive results(atypical squamous cell of undetermined significance and above this level) in cytological tests to observe the positive rate.Results(1) The respective relevance ratio of cervical abnormal cells through Thih-Prep cytolosy test and conventional smear was 8.58% and 3.75%(P<0.01).(2)The positive rate of Thih-Prep cytolosy test associated with biopsy under colposcope was 71.84%,and conventional smear was 35.56%(P<0.01).Conclusion It can obviously increase relevance ratio of cervical(especially cervical canal) abnormal cells.The combination of Thih-Prep cytolosy test,localization biopsy under colposcope and pathological test can increase the diagnostic accuracy of cervical intraepithelial neoplasia.
