| 摘 要： | 目的:探讨显微内窥镜下椎间盘切除术(MED)治疗腰椎间盘突出症(LDH)及继发病变的术中困难、并发症的对策。方法:回顾分析142例LDH的MED治疗,添制微创器械,对其术中困难、并发症进行个体化处理。结果:术中困难、并发症有:定位错误与出血过多各1例,损伤硬脊膜5例,导针穿破黄韧带3例,工作通道建立困难10例。优良率为95.7％。结论:通过针对性地完善器械,提高操作技巧,能有效地减少术中困难和并发症。

| Abstract: | Objective:To evaluate the difficulty in the treatment of lumbar discherniation using microendoscopicdiscectomy and its countermeasure. Methods: 142 cases with LDH were retrospectively analyzed. Results: Intraoperativedifficulties and complications included: 1 wrong localization and too much bleeding (1 case), damaged dura mater ofspinal cord(5 cases), perforated yellow ligament (3 cases),and different degree of difficulty in creating operative channel(10 cases). Conclusion: By individually designing and manufac turing some proper instruments and gradually improvedlevel of manipulation, the difficulties during microendoscopic discectomycould be decreased effectively. |
