孤立性纤维性肿瘤的影像学及病理学分析

目的 探讨孤立性纤维性肿瘤（solitary fibrous tumor,SFT)的影像学及病理学特征。资料与方法 回顾性分析10例胸膜及腹部SFT的影像学表现，并与病理组织学及免疫组化对照观察。结果 （1）SFT多见于胸腔，平片呈边缘光滑的孤立性肿块。（2）CT检查孤立性肿块部分可见囊性退变区，增强扫描肿瘤实质部分明显强化。（3）MRI肿瘤实质在T2WI上呈低信号，较大的肿瘤在T2WI上呈高低混杂信号且可见血管流空信号。（4）病理学检查肿瘤由梭形细胞构成，有多种排列方式，免疫组化CD34阳性。结论 SFT是一种间叶源性软组织肿瘤，影像学上见孤立性，境界清楚的软组织肿块时应考虑到该肿瘤可能，确诊须靠病理学及免疫组化检查。

Pathological and Imaging Analysis of Solitary Fibrous Tumors

Objective To study the pathological and imaging features of the solitary fibrous tumors (SFT).Materials and Methods The imaging findings of pleural or abdominal SFT in 10 patients were retrospectively analyzed, and a comparison with histopathological and immunohistochemical results was made. Results (1) SFT predominantly occurred in the thorax, being manifested on plain film as a smooth margined solitary mass. (2) On CT scans, cystic degeneration area and marked enhancement of tumor's parenchyma were demonstrated. (3) On T 2WI, most tumors were hypointense, and larger lesions displayed low high mixed signal with signal void of flow. (4) Microscopically, SFT was composed of spindle cells arranged in different patterns, with CD 34 being immunohistochemical positive. Conclusion SFT is a soft tissue tumor originated from mesenchyma. The possibility of SFT should be considered when a soft tissue mass with sharp border is seen on imaging examination. However, the verification of the diagnosis should be based on the histopathological and immunohistochemical examinations.
