中山市慢病综合防治知信行研究

目的 建立慢病综合防治知信行模型。方法 公共卫生政策分析，流行病学调查与典型相关分析。内容 知信行模式的组织基础、基本内容及关键因素具体实施、新技术应用、实施效果等。结果 通过具体社区建设，质量控制及可持续发展的配套机制建立，自动适应、自我生成、梯度推进效应逻辑模型创立，全面推动抽象社区的形成，从而推动慢病综合防治知信行模型的建立。通过模型实施，健康教育、行为干预、健康监测等实施效果显著。总体上达到或超过设计阶段国家标准水平。结论 中山古镇创立了慢病综合防治的知信行模型，且实施效果显著。

Study on the knowledge, attitude and behavior model of holistic prevention and control of chronic diseases in Zhongshan

Objective To set up the model of knowledge,attitude and behavior of holistic prevention and control of chronic diseases in Zhongshan.Methods The organization base,basic contents,new technologies and effects on knowledge,attitude and behavior model were evaluated by public health policy analysis,epidemiological survey and typical correlation.Results Through the community building,the mechanism of quality control and sustainable development was established,the logical model of automatism fit,self-creation and pushing-effect was founded,the abstract community was formed,and the model of knowledge,attitude and behavior of comprehensive prevention and control of chronic diseases was constituted.After implementing the model,the effects of health education,behavior intervention and health monitoring were obvious,reaching or excessing the national standard in the mass.Conclusion The model of knowledge,attitude and behavior of holistic prevention and control of chronic diseases was founded by Guzhen community in Zhongshan and its implementing effects were observed.
