"三镜"在胆囊结石伴胆总管结石治疗中的应用(附38例报道)

目的探讨"三镜"联合应用治疗胆管结石的疗效.方法 38例胆囊结石经ERCP检查确诊合并继发性胆管结石患者接受EST取石和腹腔镜胆囊切除术(25例),部分患者施行腹腔镜下胆囊切除和经胆囊管(5例)或胆总管切开取石术(8例)治疗.结果 38例患者成功进行"三镜"联合治疗,术后恢复快,住院时间短,无并发症,痊愈出院,随访6～12个月无残余结石和返流性胆管炎.结论 "三镜"联合治疗继发性胆管结石是安全可靠、疗效确切的微创治疗,值得推广.

Application of " three endoscopies" in the treatment of gallstone complicated bile duct stone

Objective The effect of three endoscopies for treatment of gallstones complicated bile duct stones was explored.Methods Thirty eight patients with gallstone complicated bile duct stone were determined by endoscopic retrograde cholangiopancreatography, twenty five patients were underwent for endoscopic sphinteropapillatomy and extracted stone from bile duct under endoscopy with laparoscopic cholecystectomy, five patients for laparoscopic cholecystectomy and extracted stone from bile duct stone by systic duct with bile duct endoscopy, eight patients for laparoscopic cholecystectomy and extracted stone from bile duct stone by choledochostomy with bile duct endoscopy.Results Thirty eight patients were underwent successfully combined treatment of three endoscopies. No complication occurred. These patients got the quick recovery. Following up for six months to 12 months, no evidence for residual stone and refluent cholangitis was found.Conclusion Combined appliances of three endoscopies is a mini injured therapy for gallstone complicated bile duct stone characterized safety and dependability.
