影响肝门部胆管癌预后的临床病理学多因素分析

目的分析影响肝门部胆管癌术后预后的因素。方法对1997—2003年期间经手术切除的临床病历资料齐全的肝门部胆管癌病例进行研究,选择可能影响预后的8个临床病理学因素,将其对总体生存时间的影响进行单因素分析,并通过Cox比例风险模型对其进行多因素分析。结果单因素分析显示,淋巴结转移、肿瘤组织学分级和神经浸润影响肝门部胆管癌的预后(P<0.05),Cox风险比例模型多因素分析结果显示,淋巴结转移和神经浸润是影响肝门部胆管癌切除术后预后的独立危险因素。结论影响肝门部胆管癌患者切除术后预后的独立危险因素是淋巴结转移和神经浸润。

Multivariate analysis of factors influencing prognosis of patients with hilar cholangiocarcinoma after resection

Objective To analyse the factors which influence the prognosis of patients with hilar cholangiocarcinoma after resection.Methods The clinical data of 86 patients with hilar cholangiocarcinoma receiving resection in our hospital from 1997 to 2003 were collected retrospectively and clinicopathological factors that might influence overall survival were analysed.A multivariate analysis was performed through Cox proportional hazard model.Results The single factor analysis showed that the major significant factors influencing the survival of these patients were histological type of the cancer,lymph node metastasis and perineural infiltration(P<0.05).Lymph node metastasis and perineural infiltration were found to be the independent factors influencing overall survival by multivariate analysis through the Cox model.Conclusion The most important prognostic factors for hilar cholangiocarcinoma after resection were lymph node metastasis,and perineural infiltration.
