64层螺旋CT不同后处理模式与超声心动图评侈左心室射血分数的初步分析

摘    要:目的 探索64层螺旋CT不同后处理模式获得LVEF值的临床应用价值。方法 随机选择太原市中心医院2009年1月~2013年12月临床怀疑冠心病进行冠状动脉CTA检查并行心脏超声检查的患者52例,所有CT图像均进行心脏收缩及舒张两期相重建,同时进行心动周期0~90%、间隔10%的多期相重建,重建后的数据传送至工作站进行心脏功能分析,测量LVEF值。图像由两位高年主治医师进行左心室长轴、左心室尖及二尖瓣水平校正,确定测量范围的准确性,分别利用两期相、多期相两组数据分别获得LVEF值,结果 分别记录,并由本院超声科获得同一患者的超声心动图LVEF值,以超声结果 为参照标准,与CT结果 进行对比,并进行统计学分析。结果 所有受检者均完成心脏CT及超声心动图检查。心脏CT两期相重建数据计算的LVEF值为(67.58±79.220)%,多期相重建数据计算的LVEF值为(67.75±7.869)%,超声心动图检查获得的LVEF值为(68.04±7.941)%,心脏CT两期相重建数据测得的LVEF值与超声心动图测得的LVEF值比较,差异无统计学意义(t=1.847,P=0.071);心脏CT多期相重建数据测得的LVEF值与超声心动图测得的LVEF值比较,差异无统计学意义(t=1.571,P=0.122)。结论 利用64层螺旋CT冠状动脉CTA两期相图像可以较快速的提供LVEF信息,与超声心动图结果 相比无统计学差异,具有一定的临床应用价值。

Preliminary analysis of evaluating left ventricular ejection fraction using different post processing mode of 64-slice spiral CT and echocardiography

Abstract:Objective To investigate the clinical application value of the LVEF obtained by different post processing mode of 64-slice spiral CT. Methods 52 patients suspected coronary heart disease clinically were randomly chosen in Centre Hospital of Taiyuan city from January 2009 to December 2013,and these patients were examined by CTA and echocardiography.All CT images were caught on cardiac systolic and diastolic two phase reconstruction,at the same time,and images were caught on multi phase reconstruction of heartbeat cycle 0-90%,at intervals of 10%,the data of reconstruction were transferred to the workstation for cardiac function analysis,the LVEF value were measured.The left ventricular long axis,the left ventricle tip and level of mitral valve of all images were corrected by two attending doctors to determine the accuracy of the measuring range,Using data of two phases,muhiphase,the LVEF value were measured and recorded respectively,while the same patient's LVEF value were measured in the ultrasound department of the same hospital.Using the results of ultrasound as the reference standard,comparing the results of CT,and statistical analysis was done. Results LVEF value of two phase reconstruction data of cardiac CT was (67.58±79.220)%,LVEF value of multi phase reconstruction of data calculation was (67.75±.869)%,the LVEF value of echocardiography was (68.04± 7.941)%,and there was no statistical difference the LVEF value of two phase reconstruction data of CT and echocardiography (t=1.847 ,P=0.071),and there was no statistical difference the LVEF value of multi phase reconstruction data and echocardiography (t= 1.571, P=0.122). Conclusion The LVEF information can be provided quickly Using two phase construction images of 64-slice spiral CT coronary artery CTA can be provided quickly,and it has a certain clinical application value.
