犬侧壁型囊状动脉瘤微弹簧圈栓塞前后的影像学研究

摘    要:目的 建立犬颈总动脉(CCA)侧壁型囊状动脉瘤(AN)模型,评价微弹簧圈行动脉瘤腔栓塞前后影像学的改变。方法 运用显微外科技术制作16个CCA侧壁型动脉瘤,术后2周对模型组、微弹簧圈栓塞组和对照组行彩色多普勒超声仪、经颅多普勒(TCD)、磁共振(MRI)、磁共振血管成像(MRA)、数字减影血管造影(DSA)检查并观察病理改变。结果 建立14个模型,制作成功率为87.5%,DSA结合无创影像学可以提高对动脉瘤的诊断和全面认识。微弹簧圈(MC)瘤腔栓塞能取得良好效果。结论 该模型是进行AN血管内栓塞治疗研究的较理想模型;先进的影像学检查对AN的MC栓塞前后的评价具有无创、可重复、显示清楚的特点,可以在一定程度上替代DSA检查。

Abstract:Objective To establish canine laterial saccular aneurysm model of common carotidartery (CCA) and evaluate the image changes of aneurysm cavities before and after embelization with mi-crocoil (MC). Methods 16 CCA laterial saccular aneurysm models were created by using microsurgicaltechnique. Two weeks after surgery, model group, MC embolization group and control group were subject-ed to the examinations of color Doppler, Transcranial Doppler (TCD), magnetic resonance image (MRI),magnetic resonance angiography (MRA), digital subtraction angiography (DSA). At the same time thepathological changes were observed. Results 14 Models were created with a successful rate of 87. 5%.DSA combined with other image methods could improve diagnosis of aneurysms completely. MC compact-ly pack aneurysm cavities could make good results. Conclusion This kind of aneurysm model is an idealone to be used to study the aneurysm embolization through intravascular approach. Advanced image exam-inations can noninvasively, repeatedly, clearly evaluate aneurysms before and after embolization with MC,and even substitute for DSA to some extent.
