闭合性胰腺外伤的诊治(附17例报告)

目的　探讨闭合性胰腺损伤的诊断与治疗方法。方法　回顾性分析我院 1992～ 2 0 0 2年共收治的 17例闭合性胰腺损伤患者的临床资料。结果　 17例中 ,发生术后并发症 6例 ,胰瘘 3例、胆瘘 1例、肠瘘 1例、腹腔残余感染1例 ,死亡 1例。结论　胰腺损伤早期诊断困难 ,CT是首选的检查方法 ,胰腺损伤的手术方式较多 ,应根据患者胰腺损伤的具体情况选择合理的手术方式 ,良好的围手术期管理能减少并发症和病死率的发生。

Diagnosis and treatment of closed pancreatic injury(report of 17 cases)

Objective To investigate the diagnosis and treatment of closed pancreatic injuries. Methods The clinical data of 17 patients with closed pancreatic injury who received treatment in our hospital from 1992 to 2002 were analyzed retrospectively. Results Among the 17 cases. 6 cases had developed post-operative complications, including 3 cases of pancreatic fistuia, 1 ease of bile leakage, 1 case of anastomatic leakage and 1 case of abdominal remenant infections, one patient died. Conclusion Early diagnosis of pancreatic trauma is difficult, Computed tomography (CT) examination is the first choice to its diagnosis. Different operative procedures can be used and rational methods should be chosen according to the injury degree. Excellent management of post-operation can reduce the operative complications and mortality.
