冠状动脉搭桥术后早期心律失常相关因素分析

对冠状动脉 (简称冠脉 )搭桥术后早期心律失常的相关因素进行回顾分析 ,将 6 0例冠脉搭桥病人按术后是否发生心律失常以及手术方式分组 ,心律失常组 15例 (A组 )、非心律失常组 4 5例 (B组 ) ;常规搭桥组 39例 (Ⅰ组 ) ,非体外搭桥术组 2 1例 (Ⅱ组 )。结果 :心律失常的发生率为 2 5 .0 %。A组病人的年龄、术前射血分数 (EF)、心胸比例、搭桥支数、呼吸辅助时间、心肌阻断时间、ICU滞留时间均显著高于B组 ,但血K+ 浓度则明显降低。Ⅱ组的室性心律失常发生率、心律失常持续时间、呼吸机辅助呼吸时间、ICU滞留时间均较Ⅰ组明显降低。结论 :患者术前心功能情况、手术方式及冠脉搭桥支数、心肌阻断时间等影响心脏状况的指标与其术后心律失常的发生率有关。

Analysis of the Risk Factors on Earlier Post-Operation Arrhythmia After Coronary Artery Bypass Grafting

To analyse the risk factor of earlier arrhythmia after coronary artery bypass grafting(CABG),60 patients undergone coronary artery bypass surgery were divided into two kinds of groups:arrhythmia group(group A,15 cases) and no-arrhythmia group(group B,45 cases) or CABG group(group Ⅰ,39 cases) and off-pump CABG group(group Ⅱ,21 cases). Results:the incidence of arrhythmia was 25.0%.The age was older,the ejection fraction was smaller,the heart-chest ratio was bigger and breath assist time,block time and ICU stay time were longer in group A than those in group B.The incidence of ventricular arrhythmia was higher, arrhythmia last time,breath assist time,ICU stay time were all shorter in group Ⅱ than those in group Ⅰ.Conclusion: the factors related to myocardium ischemia,such as the preoperative cardiac function,operation manier,pass-number,block time,etc,could effect the incidence of earlier post-operation arrhythmia after CABG.
