高角型双颌前突矫治前后软硬组织改变分析

目的探讨正畸治疗对高角型双颌前突患者软硬组织矢状及垂直向的影响。方法对18例高角型双颌前突患者在拔除4个第一前磨牙后,采用直丝弓矫治技术进行矫治。矫治前后拍摄X线头颅侧位定位片,并对矫治前后软硬组织侧貌的变化进行分析。结果①上下颌骨矢状及垂直向关系无明显变化;②上下前牙突度和倾斜度明显减小;③上下磨牙垂直向及矢状向支抗得到较好控制,牙合平面角无明显改变;④上下唇突度明显减小,上唇长度加长,下唇长度减小。颏厚度增加1.70mm,颏唇沟加深1.13 mm。结论高角型双颌前突患者拔牙矫治后垂直向及矢状向支抗可得到较好控制,侧貌有明显改善。

Study of changes in facial profile after orthodontic treatment in high-angle bimaxillary protrusive patients

Objective To investigate the changes of facial profile in anterior-posterior and vertical direction in high-angle bimaxillary protrusion patients after orthodontic treatment.Methods A total of 18 highangle bimaxillary protrusion patients(male 5,female 13),aged from 15.5 to 29.0(averaged 23.9 years old),were selected to be treated with straight wire technique consisting of 4 first premolars extraction.Cephalometrics were taken before and after treatment.The changes of facial profile were studied with the computer-aid X-ray cephalometric analysis.Results ①The relationship between anterior-posterior and vertical direction of the maxillary and the mandible did not change significantly.②The upper and lower anterior were more retrusive,U1-VF decreased by 6.60mm,L1-VF decreased by 5.67mm,and the tripping of incisor decreased significantly.③The anchorage of the upper and lower first molar in anterior-posterior and vertical direction were controlled preferably.Occlusal plane angle(OP-SN) did not significantly change.④The upper and lower lips were more retrusive,Ls-VF decreased by 2.93 mm,and Li-VF decreased by 3.54 mm.The length of upper lip increased significantly,while the length of the lower lip decreased significantly. Pg-Pg' increased by 1.7 mm,and Si-LiPg' increased by 1.13 mm.Conclusion The anchoragse in anterior-posterior and vertical direction were controlled preferably.Facial profiles were improved significantly after orthodontic treatment with four premotars extraction.
