带蒂大网膜填塞治疗严重肝损伤的体会

目的　探讨带蒂大网膜治疗严重肝损伤的方法和效果。方法　回顾性分析了 1995年 7月至 2 0 0 1年 6月收治的严重肝损伤应用带蒂大网膜填塞治疗的 2 5例患者。结果　采用此法治愈 17例 (治愈率 6 8% ) ,死亡 8例(死亡率 32 % )。术后再发肝出血 8例 (占 32 % ) ,膈下脓肿 9例 (占 36 % ) ,肝脓肿 2例 (占 8% )。结论　在严重肝损伤创面彻底清创止血后 ,填塞带蒂大网膜并施之一定的压力 ,是治疗肝损伤的有效方法。

Study on the treatment of serious liver trauma by packing pedicle greater omentum

Objective To evaluate the methods and effects in the treatment of serious liver trauma with pedicle greater omentum.Methods 25 cases serious liver trauma from July 1995 to June 2001 treated by using pedicle greater omentum were retrospectively analyzed.Results 17 cases were cured (cure rate 68%),8 cases were dead(mortality 32%).Secondary liver bleeding showed in 8 cases (32%),Subphrenic abscess developed in 9 cases (36%) and liver abcess appeared in 2 cases(8%).Conclusions After complete debridement and hemostasis of liver injuries,it is an efficient method to pack in the liver injuries with pressure by using the pedicle greater omentum.
