某院围手术期患者抗菌药物应用情况分析

目的了解某医院围手术期患者抗菌药物应用情况,促进临床合理用药。方法依据《应用抗菌药物防治外科感染的指导意见》及《抗菌药物临床应用指导原则》,对某院随机抽取的262例围手术期患者抗菌药物应用进行回顾性分析研究。结果262例围手术期患者100%应用抗菌药物;联合应用2种抗菌药物者69%;用法、用量不合理者30%;用药时机不合理者61%;用药疗程不合理者41%;药物选择起点过高者27%;无指征联合用药者34%;手术部位感染率4.6%。感染患者中43%未作微生物培养及药敏试验,4%未根据药敏结果适当调整抗菌药物品种。结论该院围手术期患者不合理应用抗菌药物现象比较普遍、严重,许多问题亟待解决。

Analysis on Application of Antimicrobial Agents in Perioperative Patients

OBJECTIVE:To analyze the application of antimicrobial agents in perioperative patients in a hospital so as to promote their rational use in the clinic.METHODS:The application of antimicrobials in 262 perioperative patients randomly selected from a hospital was reviewed retrospectively based on the Guidance on the Application of Antimicrobials for the Prevention of Surgical Infection and the Clinical Application Guideline on the Antimicrobials. RESULTS:Of the 262 cases,100% received antimicrobial agents,69% received combination of 2 drugs,30% were irrational in usage and dosage,61% were irrational in medication time,41% were irrational in medication duration,27% involved high starting point of drug application,34% received drug combination yet without any indication,4.6% involved infection of operative sites. Of those who were subjected to infection,43% received neither microbiological culture nor susceptibility test,and 4% didn’t adjust the categories of antibiotics accordingly based on the results of susceptibility test.CONCLUSIONS:The phenomenon of irrational use of drugs in perioperative patients of this hospital is widespread and serious,and there are many problems needed to be tackled urgently.
