摘    要:目的探讨下肢骨折患者血浆D-二聚体(D-dimer,DD)与血糖、血脂水平的相关性及其临床意义。方法使用全自动血凝仪检测100例骨科住院下肢骨折病例伤后7~10天血浆DD水平,使用全自动生化仪检测同期空腹血糖(fasting plasma glucose,FPG)、低密度脂蛋白胆固醇(low densitylipoprotein-cholesterol,LDL-C)水平。结果 100例患者81例DD阳性,其中12例患者被确诊为发生深静脉血栓(deep vein thrombosis,DVT)。DD阳性组FPG、LDL-C水平显著高于DD阴性组(P〈0.05);DD阳性患者中DVT阳性组DD、FPG水平显著高于DVT阴性组(P〈0.05),LDL-C水平无统计学差异(P〉0.05)。下肢骨折患者血浆DD水平与FPG、LDL-C水平成显著正相关(r=0.512、0.289,P〈0.05)。结论高血糖、高血脂可能增加下肢骨折患者DVT的发生率。

Abstract:Objective To investigate the correlation of the changes of plasma D - dimer (DD) with levels of plasma glucose and lipid in inferior extremity fractured patients and to explore the clinical significance. Methods Seven to ten clays post fracture in 100 inferior extremity fractured patients, the level of DD was detected by automatic coagulation analyzer; the levels of fasting plasma glucose (FPG) and low density lipoprotein - cholesterol ( LDL - C) were assayed by automatic biochemical analyzer. Results 81 cases were DD positive in which twelve cases were suffered from deep venous thrombosis (DVT). The levels of FPG and LDL - C in DD - positive group were significantly higher than those in DD - negative group ( P 〈 0. 05 ). In the DD - positive group although there was no significant difference for the LDL - C level ( P 〉 0. 05 ) , the levels of DD and FPG in patients with DVT were significantly higher than those in DVT negative patients ( P 〈 O. 05 ). The level of plasma DD was significant positively correlated with FPG ( r = 0. 512, P 〈 0. 05 ) and LDL - C ( r = 0. 289, P 〈 0. 05) levels in inferior extremity fractured patients. Conclusions The aberrant blood glucose and lipid could promote the pathogenesis of DVT in patients with inferior extremity fracture.
