不同骨受植床游离植骨成骨能力比较的实验研究

摘    要:目的:探讨保留髁状突的双端骨受植床和单端游离的骨受植床在游离植骨中新骨形成的能力。方法:建立单端或双端骨受植床的兔动物模型,同期自体髂骨移植修复。术后第2、4、8周处死各组动物,测量升支高度并进行骨组织形态学定量分析。结果:双端骨受植床组术侧升支高度与健侧相比无显著性差异(P>0.05);单端受植床组术侧升支高度与健侧相比有显著性差异(P<0.05),术侧明显降低;骨组织形态定量学评价显示双端骨受植床组各时段上下两条骨愈合线新骨形成能力无显著性差异(P>0.05);两组间比较术后各时段组织形态定量分析均无差异,但双端骨受植床组上下两条愈合线相加的绝对值几乎是单端组的2倍。结论:保留髁状突的双端骨受植床在保持升支高度以及游离植骨的骨愈合方面明显好于未保留髁状突的单端骨受植床病例。

Abstract:Objective:The aim of this study is to compare the osteogenic ability of the unilateral bony recipient site and bilateral bony recipient site in free bone graft, and to provide experiment data for clinical therapy. Method: Using rabbit's mandibular ramus to make animal model. One team keep the condylar process, the other excise it. Illac graft restore the defection, then observe the healing of the bony graft. Though measure the highness of the experiment and the control groups' mandibular ramus, to evaluate the clinical effect;Through bone histomorphometry to assess the ostogenic ability.Result:The highness of mandibular ramus of the bilateral recipient site reduce less than the unilateral's, there is statistical signification. There is no statistical signification in the bone histomorphometry, thought the quantity of new trabeculr in former group is more than in the latter. The vessel density in two group haven't statistical differentia. The rate of the new bone formation in the bilateral site group is better than the unilateral site group.Conclusion:In the process of clinical therapy, it should keep the condylar process as possible. Thus will maintain the highness of mandibular ramus, which is important in restoring the function of mandibular. Meantime, the bilateral recipient site provide favorable fixation, which is important to the survive of the graft, and reduce the quantity of the donor bone .
