鞍区肿瘤与肿瘤样病变的影像诊断

目的探讨鞍区肿瘤与肿瘤样病变的CT、MRI影像表现及鉴别诊断。方法回顾性分析127例鞍区肿瘤与肿瘤样病变的CT、MRI平扫及其增强扫描影像表现，并与手术证实和病理结果对照。结果不同病变在生长部位、信号或密度上各有特点，大部分病例影像检查能做出正确定性、定位诊断，部分病例信号或密度相似，鉴别诊断有一定困难。结论MRI是鞍区病变首选检查方法，能很好显示病变大小、形态及邻近解剖关系，显示血管及血管病变也明显优于CT，但CT在显示病变有无钙化及骨质破坏程度方面是MRI不能替代的，对鞍区病变做出正确诊断还应结合增强扫描及临床资料。

Image Diagnosis of Tumor and Tumor-like Lesions in sellar Region

Objective To discuss the CT and MRI image manifestation and the differential diagnosis of sellar tumor or tumor-like lesions. Methods A retrospective analysis was made to 127 plain and enhancement scans Of CT and MRI of sellar tumor or tumor-like lesions and the images were compared with the pathological reports after operation. Results Different cases had their own features in site, signal or density. Most of the images provided correct qualitatlon and location while some cases showed out similar signals or density, which caused certain difficulties for differential diagnosis. Conclusions MRI is the first choice to examine the sellar lesions, it can well display the size, shape and the surrounding anatomic state of the lesions, MRI is superior to CT in displaying blood vessels and vascular lesions, but to display calcification or destruction of bones, CT is more superior to MRI. Contrast enhanced scan and clinical data should be combined with the images in diagnosing the sellar lesions correctly.
