精液粘度对精液质量的影响及相关因素分析

目的：观察精液粘度增高对精液质量的影响及相关因素分析。方法：308名男性不育门诊患者，按精液粘度正常与否分为粘度增高组和正常组，进行精液检查。结果：精液粘度正常组和增高组患者的精液量、精子密度、a级精子百分比、b级精子百分比、c级精子百分比、正常形态精子百分比、精子运动轨迹各项参数之间无统计学差异，P＞0.05；粘度增高组精液pH值、液化时间、d级精子百分比分别为7.3±0.2、22.0±4.3（min）和（59.5±15.5）%，明显高于粘度正常组，分别为7.1±0.2、20.7±5.1（min）和（54.5±19.3）%，P＜0.05；粘度增高组精子存活率为（65.2±17.0）％，明显低于粘度正常组，后者为（70.7±15.2）％，P＜0.05。精液粘度增高与精液白细胞数量无关（rs=-0.005，P值=0.927），与精浆抗精子抗体存在正相关（rs=0.184，P值=0.001）。结论：精液粘度增高可以导致精液液化时间延长，pH值增高，降低精子存活率和活动力，与精浆抗精子抗体相关，应当引起临床医生的高度重视。

The effect of semen viscosity on semen quality and analysis of related factors

Objective To investigate the effect of seminal hyperviscosity on semen quality and analyze the related factors.Methods 308 infertile men were divided into hyperviscosity group and normal viscosity group according to the state of semen viscosity,then conventional semen analysis was performed.Results The semen volume, sperm concentration,the percentage of a,b and c grade motile sperm, the percentage of normal sperm morphology and parameters of sperm movement were no significant difference between these two groups, P＞0.05;The semen PH value ,liquefaction time and percentage of d grade sperm in the hyperviscosity group were 7.3±0.2, 22.0±4.3（min）and （59.5±15.5）% respectively,markedly higher than that in the normal viscosity group,which were 7.1±0.2, 20.7±5.1（min）and (54.5±19.3）% respectively,P＜0.05;The sperm survial rate in the hyperviscosity group was（65.2±17.0）％,differently lower than that in the normal viscosity group, which was（70.7±15.2）％,P＜0.05. Semen viscosity increase had nothing to do with the number of semen leukocyte,rs=-0.005，P value =0.927;There was positive relationship between seminal hyperviscosity and antisperm antibody, rs=0.184，P value =0.001.Conclusions Seminal hyperviscosity can lead to increased semen liquefaction time and pH value, reduce sperm survival rate and motility, be related to antisperm antibody ,and shoud be attached great attention.
