Narcotrend监测下七氟醚吸入全麻辅助右美托咪定的临床观察

目的观察在NarCotrend监测下七氟醚吸入全麻辅助右美托咪啶在妇科腹腔镜手术中的麻醉效果及安全性。方法选择40例择期妇科腹腔镜手术患者随机分为右关托咪啶＋七氟醚（DS组，n=20）和生理盐水＋七氟醚（NS组，n=20）两组．DS组于诱导前10min静脉泵注右美托咪啶0．70μg／kg，0．4μg·kg-1·h-1维持麻醉直到手术结束，NS组静脉泵注相同容量的生理盐水。结果七氟醚用量Ds组为（19．63±2．5）mL，Ns组为（38．4±6．2）mL，两组七氟醚用量比较，差异有统计学意义（P〈0．01）：两组组间NI及MAP不同时间点比较均无明显差异（P=0．000）；两组组间HR及Cetscvo不同时间点比较有明显差异（P〈0．005）。结论右美托咪啶能够加强七氟醚的麻醉效果并有效节俭七氟醚的量，能安全辅助应用于七氟醚吸入全身麻醉。

Clinical Observation of Dexmedetomidine as an Adjuvant to General Anesthesia of Sevoflurane Inhalation with Narcotrend Monitoring

Objective To investigate the efficacy and safety of dexmedetomidine in gynecological laparoscopic surgery for general anesthesia of sevofiurane inhalation with Narcotrend monitoring. Methods 40 female adults were randomly divided into two groups： dexmedetomidine and sevoflurane （DS group, n =20）, saline and sevoflurane （his group, n = 20）. The DS group received dexmedetomidine 0.70 μg/kg over 10 min followed by 0.4μg. kg-1. h4 until the end of operation. Saline intravenous was given at the same capacity in the NS group. Results The volume of sevoflurane was （19.63 ＋ 2.45） mL in DS group and （38.37 ±6.17） mL in NS group （P 〈0.01）. NI and MAP had significant difference at different time points between the two groups （P = 0.000）, HR and Cetsevo had significant difference at different time points between the two groups （P 〈0.005）. Conclusions Dexmedetomidine can enhance the effect of sevoflurane anesthesia and reduce the consumption of sevoflurane; it is safe to be used as an adjunct to general anesthesia of sevoflurane inhalation.
