上海市公立性病门诊STD服务质量调查

目的了解上海市三家公立性病门诊性传播疾病(STD)服务质量指标2(包括安全套促进、建议性伴通知和自愿咨询检测三个分指标)的执行情况,为接下来的干预提供建议。方法用定性和定量相结合的方法对性病门诊的服务质量指标2进行评估。结果三家门诊安全套促进提供率分别为:市级44.27%,区级33.01%,地段最差,仅为26.01%。几乎没有病人得到安全套使用方法的建议和免费提供。性伴通知提供率分别为:市级门诊55.28%,地段48.01%,区级仅为29.13%。大部分医务人员对性伴通知的概念理解不准确。区级和地段门诊没有将艾滋病作为重点宣教内容。市级门诊只有34.15%的病人得到有关艾滋病病毒(HIV)自愿咨询检测的建议,区级门诊为33.01%,地段仅为16.01%。结论三家性病门诊STD服务质量指标2的工作质量较差,应加强对性病门诊医务人员的培训和与计划生育部门的合作。

Quality of STD care service in public STDs clinics in Shanghai

Objective To evaluate the quality of STD care service in three public STD clinics in Shanghai in respect with performance of indicator 2 of STD care service(promotion of condom use,partner notification and VCT)and provide suggestion for further interventions.Method Condom promotion,partner notification and VCT services were assessed by both qualitative and quantitative methods.Results The rates of condom promotion in STD clinics at the municipal,district and community levels were 44.2%,33.01% and 26.01%,respectively.Few patients received free condoms or instructions for condom use.The rates of partner notification were 55.28%,29.13% and 48.01% in the clinics at the three levels,respectively.Majority of medical staff had poor or wrong understanding of partner notification;moreover,district and community clinics failed to take HIV/AID as their priority of public education.The rates of patients receiving VCT suggestions from medical staff were 34.15%,33.01% and 16.01% in the three level clinics,respectively.Conclusion Condom promotion,partner notification and VCT services were poorly performed in the three STD clinics.It will be important to strengthen staff training in STD clinics and the collaboration with the parenthood planning units.
