| 摘 要： | 目的:探讨直肠癌旁移行黏膜(TM)病理学性质及其对患者预后的影响。方法:2000-01/2001-01应用免疫组化及粘液组化方法观察了81例直肠癌及癌旁黏膜的增殖细胞核抗原(PCNA)表达情况及癌旁TM的变化规律。结果:在癌旁TM、非典型增生及癌组织PCNA呈递增表达,与正常粘膜相比,差异非常显著(F=8.92,P<0.01),癌旁TM范围在粘液癌明显大于乳头状癌及管状腺癌(t=3.15-3.41,P<0.01),在DukesC期明显大于DukesA、B期(t=2.51-3.10,P<0.05,P<0.01)。结论:直肠癌旁TM具有一定的恶性潜势,直肠癌旁TM的范围与直肠癌的类型及进展密切相关,有利于对患者预后的评估。 |

| Abstract: | AIM:To study the pathological characteristics of the transitional mucosa (TM) adjacent to rectal carcinoma and their significance in prognostic assessment of the patients. METHODS: Immunohistochemical and mucin histochemical methods were employed to observe the expression of PCNA and the changes in the TM adjacent to the carcinoma in 81 cases of rectal carcinoma. RESULTS: PCNA expression was the highest in the cancer foci and gradually decreased in atypical dysplasia, TM and normal mucosa (F=8.92, P< 0.01). The distribution of TM adjacent to the mucinous adenocarcinoma was more extensive than that to the papilla adenocarcinoma and tubular adenocarcinoma (t=3.15- 3.41, P< 0.01), and the TM adjacent to the rectal carcinoma in Dukes C stage was more extensive than that in stages A and B (t=2.51- 3.10, P< 0.01, P< 0.05). |
