肺结节少见CT征象对病变的诊断价值

摘    要:目的探讨肺结节的少见CT征象,提高周围型肺癌的诊断正确率。方法回顾分析经手术病理或临床治疗证实的186例肺内结节(139例周围型肺癌和47例其它肺内结节)的少见CT征象(多囊状透亮影、肿瘤胸膜侧的模糊小片状影、磨玻璃影)在肺癌和其它结节中的出现率。结果多囊状透亮影和肿瘤胸膜侧模糊小片状影在肺癌中出现率明显高于肺内其它结节(P<0.05)。磨玻璃影在肺癌和其它结节的出现率无显著差异。在肺癌中,磨玻璃影在细支气管肺泡癌中的出现率明显高于其它类型肺癌(P<0.001)。结论多囊状透亮影和肿瘤胸膜侧模糊小片状影对肺癌的诊断有较高价值,肺癌中磨玻璃影的出现常提示细支气管肺泡癌的可能。

Abstract:Objective To study the rare CT features in pulmonary nodules,so that to elevate the diagnosis of peripheral lung carcinoma. Methods The occurrence ratio of rare CT features(including multi-cyst lucency, undefined patchy opacity between the tumor and the pleura and ground-glass opacity) were analyzed in 139 peripheral lung carcinomas and 47 other types of pulmonary nodules retrospectively. All cases were confirmed by surgical operation-pathology or clinic. Results The ratio of multi-cyst lucency and undefined patchy opacity between the tumor and the pleura were different between peripheral lung carcinoma and other pulmonary nodules,which in the later was higher than that in the former (P<0.05). There was no difference of the occurrence ratio of ground-glass opacity between peripheral lung carcinoma and other pulmonary nodules.The occurrence ratio of ground-glass opacity in bronchioalveolar carcinoma(BAC) was higher than that in other types peripheral carcinoma (P<0.001).Conclusion The multi-cyst lucency and undefined patchy opacity between the tumor and the pleura in pulmonary nodules predict peripheral lung carcinoma. The ground-glass opacity in peripheral lung carcinoma predicts BAC.
