NCPAP联合肺表面活性物质治疗HMD32例临床分析

目的观察鼻塞式持续气道正压通气（NCPAP）治疗新生儿肺透明膜病（HMD）的疗效，以及与常规机械通气治疗新生儿肺透明膜病（HMD）的疗效和不良反应的比较。方法将60例新生儿肺透明膜病患儿随机分成两组，其中实验组32例采用常规治疗加鼻塞式持续气道正压通气加肺表面活性物质（PS），对照组28例采用常规治疗加常规机械通气加PS，比较治疗前后临床症状、PaO2和SPO2的变化以及呼吸道感染、呼吸机相关性肺损伤和支气管肺发育不良发生率。结果两组在发绀、呼吸困难、PaO2和SPO2的改善上无明显差异，但是NCPAP联合PS的呼吸道感染、呼吸机相关性肺损伤发生率明显减低。结论NCPAP联合PS在治疗新生儿肺透明膜病中有良好的疗效，与常频机械通气组比较，并发症少。

Effect of nasal continuous positive airway pressure combined with pulmonary surfactant on hyaline membrane disease

Objective To investigate the effect of nasal continuous positive airway pressure （NCPAP） combined with pulmonary surfactant（PS） on hyaline membrane disease（HMD）. Methods 60 patients with HMD were divided into two groups, 32 cases with HMD were treated with common treatment and NCPAP combined with PS, and 28 cases were treated with common treatment and conventional ventilation combined with PS. The change of clinical effects, the blood gas, and the rate of respiratory infection, VALI and BPD were observed. Results Cyanosis, respiratory distress and blood gases improved after treatment in the two groups without significant difference between the two groups. The rates of respiratory infection and VALI in NCPAP group were lower than that of conventional ventilation group. Conclusion NCPAP combined with PS has a good effect for HMD, with less complications.
