前置胎盘妊娠结局的临床分析

目的：探讨前置胎盘（ placenta previa， PP）的类型及处理措施与妊娠母儿结局的关系。方法 回顾性分析2012－02～2014－02间于我院住院分娩的51例PP患者的临床资料，根据产前超声初步诊断与产后检查胎盘明确诊断将其分为完全性PP组，部分性PP组和边缘性PP组并比较三组PP产科危险因素及母儿结局。结果 完全性PP与部分性PP的剖宫产率高于边缘性PP，三种PP的胎盘植入、产后出血及子宫切除差异有统计学意义（P＜0．05），完全性PP新生儿早产率高于另两组，出生体重小于另两组，差异有统计学意义（P＜0．05）。结论 PP的类型与妊娠结局关系密切。凶险性前置胎盘胎盘是导致产后出血及子宫切除的重要原因，积极控制出血是改善母儿预后的关键措施。

Clinical Analysis of Pregnancy Outcomes in Patients wtih Placenta Previa

Objective To explore the relationship between the types of placenta previa （ PP ） and pregnancy delivery outcomes. Methods A retrospective analysis of 51 patients with PP deliveries in the hospital from February, 2012 to February, 2014 was conducted. According to prenatal ultrasound diagnosis and examination of the placenta after delivery, and the 3 groups of PP were classified under com-plete PP, partial PP and marginal PP.The study compares these three groups of PP in relation to the outcome of child deliveries.Results Cesarean section rates were higher in complete PP and partial PP than in marginal PP .There was statistical significance among prepose placenta implantation, postpartum hemorrhage and uterus resection.Premature delivery rate of complete PP new born babies was higher than the other two groups, and the born weight was less than the other two groups, with statistical significance.Conclusion PP conditions are closely related to pregnancy outcomes.Aggressive PP conditions are the main reasons behind postpartum hemorrage and hysterectomy . Timely termination of pregnancy and control of postpartum hemorrage are critical measures to be taken in these cases.
