射频球襄成形术预防动脉腔内成形术后再狭窄的实验研究

摘    要:本文将射频球囊成形术应用于动脉粥样硬化狭窄模型,观察球囊表面电极直接释放射频能量并结合压力对动脉粥样硬化狭窄的作用,同时以单纯球囊成形术为对照。结果表明,射频球囊成形术后血管狭窄程度从平均60%以上降低至平均17%以下,明显低于对照组(P<0.05),术后4周再狭窄程度明显低于对照组。超微结构也显示,射频球囊成形术后血管平滑肌细胞增殖数量减少,提示该项技术对防止动脉腔内成形术后再狭窄可能是一项有希望的措施。

Abstract:Percutaneous transluminal balloon angioplasty(PTA) is an important and effective treatment of peripheral and coronary vascular diseases. However, the overall efficacy is limited by the relatively high freqency of postangioplasty ressenosis(PARS). In this experiment,the iliac arteries of rabbits were denuded by balloon catheter and the animals were then fed high fat diet for 5 weeks. Radiofrequency balloon angioplasty(RFBA) was applied to the atherosclerotic stenosis site and the rates of restenosis evaluated by angiography at 1( n= 10), 2 (n= 10), 3 (n= 9),and 4 (n= 9) weeks after angioplasty were compared with those of simple PTA(n= 10, 9, 8 and 8, respectively ). The rabbits were then sacrificed , and the related arteries were studied pathologically. The results revealed lower residual stenosis in RFBA than in PTA (p< 0. 05). The average rates of restenosis was lower in RFBA than in PTA at 2 to 4weeks(p< 0. 05). Transmission electron microscopy in dicated that the organells of the vascular smooth muscle cells(VSMC) of the arteries following RFBA showed denaturation and the the volume density of which was significantly diminished, with the number of synthetic SMC decreased sharply and some SMC displayed denaturation and necrosis.
