勿需缝合的黑痣切除术

为了在暴露部位尤其是面部切除黑痊获得最佳的美容效果及最小的瘢痕，从七十年代开始偿试一种切除病变后依靠创缘的自然收缩来闭锁创面的办法。手术时沿黑痣边缘圆形切开皮肤全层，压迫止血后，用无菌敷料敷盖创面，不需任何其他处理，创面即可在术后１０天至４周愈合。创面越小，愈合时间越短。此间如创面内出现凝血块，应及时取出，因为凝血块会妨碍创缘的收缩。创面愈合不久会出现色素沉着，但最终一定会消失，这种手术适合于小于１０ｍｍ的病变，其优点是瘢痕小，手术简便，但因为创面开放，因此容易并发感染。

The resection of melanotic nevus without suture

In order to obtain the best esthetic result after the removal of melanotic nevus,a new method was developed in 1970,which wounds could heal without suture The adaptable sites to the op-eration are eyebrow,eyelid,philtrum,vennilion bordr, nasolabial sulcus,and ala nasi.The suitable size of above wound is no more than 10mm in diameter.In addition to melanotic mevus,the indication also includes the superficial skin tumors,small scar and pigmentation with the same size.The lesion must be resected in circular shape deep to the subcutaneous layer.After removal of melanotic nevus, the wound is pressed for hemostasis and then is bandaged with gauze to keep it from contamination.The woundwill heal in no more than four weeks according to its size. The open full-thickness wound could heal bynature contraction but suture.Soon later,the pigmentation will happen.How long it will be is differentin individual,but it must disappear.Smaller scar is the advantage of this method over " regular sutuermethod".That open wound is subjectable to infection is its shortcoming.
