原位肝移植的麻醉处理

目的 探讨肝移植麻醉的术中处理。方法 对4例终末期肝病患者在全麻下实施肝移植手术过程中的麻醉处理，术中有创动脉压、上下腔静脉压、尿量、脉搏氧饱和度(SpO2)、血气、电解质、血常规和凝血功能监测及相应的处理措施进行回顾性分析。结果 4例手术均顺利完成手术，术中病人生命体征平稳，新肝开放过程顺利，提示麻醉、监测及相应的处理措施得当。结论 肝移植麻醉中，维持麻醉和循环稳定及多系统多器官的功能监测是保证手术成功的关键。

Anesthesia management for orthotopic liver transplantation

Objective To approach the anesthesia mana ge ment for liver transplantation during operation.Methods T o analysis ,retrospectively, the anesthesia management and invasive arterial blo od pressure and upper and inferior vena cava pressure and urine volume and pulse oxygen saturation (SpO 2) and blood gas and electrolyte blood routine and thro mboxane function monitor and related dealing measures of 4 cases with terminal s tage of liver illness who underwent orthotopic liver transplantation under gener al anesthesia.Results It showed that the 4 cases successf ully underwent the operation and life signals kept stable and the new liver enab ling procedure was smooth, and all of these indicated that the anesthesia manage ment,the monitor and the related dealing measures were suitable.Conc lusion It shows that keeping anesthesia and circulation system stabl e is the key to guarantee the operation is done successfully.
