道路交通事故伤的紧急救治（附84例临床分析）

目的：总结交通事故伤的救治经验，提高抢救成功率。探索急诊创伤外科组建的必要性。方法：对近3a来84例重症交通事故伤的抢救治疗情况进行总结分析。按照紧急生命评估程序边诊断边救治。创伤严重程度按照AIS－ISS标准评分。救治中除常规处理外，多采用限制性、亚低温输液法补充血容量，并采用适当药物治疗。结果：抢救成功51例（61％），死亡33例（39％）。死亡原因的第一位为颅脑损伤（48．5％）；第二位为出血性休克（30．3％）。结论：交通伤在伤后1h得到救治有望提高伤者的生存率。另外，现场急救和转运监护措施得当，综合医院拥有训练有素的急救队伍，是提高交通事故伤救治水平的重要因素。

The emergency treatment of road traffic accidents (clinical analysis of 84 cases)

Objective:To raise the rate of successful emergency treatment by summing up the experience of emergency treatment in severe road traffic accidents and approach to the necessary of organizing the system of emergency treatment medicine.Methods:The clinnical data of 84 cases of severe road traffic accidents in recent 3 years were reviewed.Accordng to the Abbreviate Injury Score(AIS)and Injury Severe Score(ISS),diagnosis and treatment were done simultaneously.Fluid replacement by putting a limit on quantity and in moderate temperature was always taken except general management.Results:There were 33 patients died from severe road traffic accidents.The mortality was 39%,the rate of succesful treatment was 61% in this group.The first cause of death was the trauma of the brain,which resulted in death of 16 patients (48.5%),the second cause of death was shock of hemorrhage(30.3%).Conclusion:The survival rate of road traffice patients could be improved in 1 hour after trauma by emergency treatment.Besides this,it is important to enhance the first aid and transport of patients in monitoring and to train the team of emergency medicine in general hospital for the emergency treatment of road traffic patients.
