低流量循环紧闭麻醉时异氟醚耗用量的临床研究

目的:探讨全程低流量循环紧闭麻醉时异氟醚的耗用量。方法:35例ASA Ⅰ～Ⅲ级需行气管插管全麻的择期手术患者,手术时问均在2h以上,术中均无大失血。麻醉诱导后,在吸入端螺纹管与麻醉机的连接处插入5号细针头,接上三通,通过此途径经微量泵持续气道内给异氟醚。记录病人每h异氟醚的耗用量及各时段的CI、CE并计算异氟醚的CI—CE及CI—CE/CE。结果与结论:异氟醚的耗用量,第1h耗用量(9.68±1.78)ml最多,和其它各时段相比有非常显著性差异(P<0.01);第2h耗用量(7.49±1.59)ml多于第3h(5.79±1.16)ml和第4h(5.72±1.28)ml(P<0.01);第3h与第4h相比无显著性差异(P>0.05)。这为麻醉医师于术毕准确、客观地估计病人围术期异氟醚的耗用量提供了一个可靠的参考指标。

A Clinical Study of Isoflurane Consumption during Low-flow Closed Circuit Anesthsia (LFFCCA)

Objective　To investigate the isoflurane consumption during LFCCA. Methods　Thirty-five patients with ASA of grade Ⅰ～Ⅲ underwent the elective surgery lasting longer than 2 hours under general anesthesia through the tracheae. No massive hemorrhage occurred during the operations in all the patients. The patients continuously received transtracheally isoflurane by the infusion pump after the endotracheal intubation. Results　The isoflurane consumption within the first an hour was 9.68 ml±1.78 ml, which was significantly more than that within other any an hour (P<0.01). In the second an hour it was 7.49 ml±1.59 ml, which was significantly more than that 5.79 ml±1.16 ml in the third an hour and that 5.72 ml±1.28 ml in the fourth an hour (P<0.01). The isoflurane consumption had no significant difference between the third and fourth an hour. Conclusion　The present data provided a reference index for an anesthesiologist estimating the isoflurane consumption during LFCCA.
