前列腺增生症术后尿路感染患者血清炎症因子、miRNA-15a、miRNA-34b和miRNA-103及CYP1A2基因多态性研究

摘    要:目的探讨前列腺增生症术后尿路感染患者血清炎症因子、微小RNA(microRNA,miRNA)-15a、miRNA-34b和miRNA-103及CYP1A2基因多态性相关性。方法选择浙江省医疗健康集团杭州医院于2017年7月-2019年7月收治的前列腺增生症患者112例,均行经尿道前列腺等离子双极电切术(Transurethral plasmakinetics resection of prostate,TUPKRP),根据是否发生术后尿路感染分为感染组(n=27)与非感染组(n=85)。分析两组患者血清炎症因子,外周血miRNA-15a、miRNA-34b和miRNA-103表达,及CYP1A2基因多态性变化。结果感染组血清CRP、TNF-α、IL-6和PCT水平高于非感染组(P<0.05)。感染组外周血miRNA-15a表达低于未感染组,而miRNA34b和miRNA103表达高于未感染组(P<0.05)。感染组CYP1A2基因位点rs2069514-3859等位基因A分布高于非感染组,而G分布低于非感染组(P<0.05)。感染组CYP1A2基因位点rs20695143859基因型AA分布高于非感染组(P<0.05)。结论前列腺增生症术后尿路感染患者血清炎症因子CRP、TNF-α、IL-6和PCT水平升高,血miRNA-15a低表达及miRNA34b和miRNA103高表达参与了术后尿路感染发生、发展,且CYP1A2基因位点rs20695143859等位基因A可能为术后尿路感染易感基因。

Abstract:OBJECTIVE To investigate the correlation between urinary tract infection and serum inflammatory factors,microRNA(microRNA)-15 a,microRNA-34 b,microRNA-103 and CYP1 A2 gene polymorphisms in patients after prostate hyperplasia surgery.METHODS A total of 112 patients with benign prostatic hyperplasia who were admitted to Hangzhou hospital of Zhejiang medical and Health Group and underwent transurethral resection of the prostate plasma(TUPKRP)from July 2017 to July 2019 were recruited.They were divided into the infected group(n=27)and the non-infected group(n=85)according to whether postoperative urinary tract infection occurred.The serum inflammatory factors,the expression of microRNA-15 a,microRNA-34 b and microRNA-103 in peripheral blood and the polymorphism of CYP1 A2 gene were analyzed.RESLUTS The levels of CRP,TNF-α,IL-6 and PCT in the infected group were significantly higher than that in the non-infected group(P<0.05).The expression of microRNA-15 a in peripheral blood in the infected group was significantly lower than that in the non-infected group,whereas the expression of microRNA 34 B and microRNA 103 was significantly higher than that in the non-infected group(P<0.05).The distribution of CYP1 A2 locus rs2069514-3859 allele A in the infection group was significantly higher than that in the non-infected group,whereas the distribution of G in infected group was significantly lower than that in the non-infected group(P<0.05).The genotype AA distribution in CYP1 A2 locus rs20695143859 in the infected group was significantly higher than that in the non-infected group(P<0.05).CONCLUSION The levels of serum inflammatory factors CRP,TNF-α,IL-6 and PCT in patients with benign prostatic hyperplasia after urinary tract infection were significantly elevated.The low expression of blood miRNA-15 a and the high expression of miRNA34 b and miRNA103 were involved in the occurrence and development of urinary tract infection.CYP1 A2 allele A can be a susceptible gene for urinary tract infection after operation.
