2型糖尿病合并活动性肺结核患者空腹血糖水平与痰培养的相关性

摘    要:目的探讨2型糖尿病合并活动性肺结核患者空腹血糖水平与痰菌培养结果的关系。方法回顾性分析2013年1月—2016年12月在深圳市第三人民医院住院的159例2型糖尿病合并活动性肺结核的患者。将患者按照痰培养结果分为两组,比较人口学及空腹血糖水平的差异,并采用Spearman秩相关分析患者年龄、性别及空腹血糖水平与痰菌培养结果的相关性。采用二元Logistic回归分析探讨痰培养结果阳性的危险因素。使用分层交互检验分析不同年龄段及性别分层时,空腹血糖水平对痰培养结果的影响。结果痰培养阴性组30例(18.9%),平均年龄和空腹血糖分别为(55.37±12.70)岁和(13.18±3.81)mmol/L。痰培养阳性组129例(81.1%),平均年龄和空腹血糖分别为(48.4±9.70)岁、(15.00±4.39)mmol/L。与痰培养阴性组相比,阳性组患者发病年龄较小(P=0.008),空腹血糖水平较高(P=0.038),性别差异无统计学意义(P=0.959)。相关性分析结果表明,痰培养结果与性别无明显的相关性(r=0.004,P=0.959),与年龄呈负相关(r=-0.133,P<0.05),与空腹血糖水平呈正相关(r=0.161,P<0.05)。Logistic回归分析表明空腹血糖与年龄是痰培养阳性的影响因素。分层分析表明,当年龄<50岁或性别为男性时,空腹血糖水平对痰培养结果的影响有统计学意义。结论2型糖尿病合并活动性肺结核患者空腹血糖高低与痰培养结果关系密切,空腹血糖水平可能是预测2型糖尿病合并活动性肺结核患者痰培养阳性的重要危险因素。

Abstract:Objective To explore the relationship between fastingblood-glucose levels and sputum culture in patients with type 2 diabetes mellitus and active pulmonary tuberculosis. Methods A total of 159 patients who were diagnosed as type 2 diabetes mellitus and active pulmonary tuberculosis in the Third People's Hospital of Shenzhen from January 2013 to December 2016 were enrolled.All patients were divided into 2 groups according to the sputum culture result. The general data and fasting blood-glucose levels were compared between the two groups. Spearman correlation analysis was used to analyze the correlation of age, gender,fasting blood-glucose levels and sputum culture. The risk factors of positive culture results were analyzed by binary logistic regression. Hierarchical interaction analysis was used to analyze the effect of fasting blood glucose level on sputum culture results in different age and gender groups. Results In the sputum culture negative group (30 cases, 18.9%), the mean age and fasting blood glucose were (55.37±12.70) years and (13.18±3.81) mmol/L, respectively. In the sputum culture positive group (129 cases, 81.1%), the mean age and fasting blood glucose were (48.4±9.70) years and (15.00±4.39) mmol/L. Compared with those with sputum culture negative,patients with sputum culture positive had lower age (P =0.008)and higher fasting blood glucose level (P =0.038), there was no significant difference in gender(P =0.959). In sputum culture group, the result was nocorrelation with gender (r =0.004,P =0.959),but negative correlation with age (r =-0.133, P <0.05) and positive correlation with fasting blood glucose level (r =0.161,P <0.05). Binary logistic regression analysis found that hyperglycemia and age were risk factors for positive sputum culture.Interaction analysis showed that the effect of fasting blood glucose level on sputum culture results was statistically significant only when the age was <50 years old or the gender was male. Conclusion Fasting blood glucose level is closely related with sputum culture result and may be an important risk factor of sputum culture positive in patients with T2DM and active TB.
