河南省HIV抗病毒治疗耐药性研究

目的:了解河南省艾滋病患者抗病毒治疗效果及耐药毒株的出现对其治疗产生的影响。方法:在艾滋病人集中的3个县作为研究现场,选择治疗和未接受抗病毒治疗的HIV感染者为对象,进行个人访谈及随访填写调查表;测定CD4+淋巴细胞计数的变化,观察血浆病毒载量评价抗病毒治疗效果;用RT-PCR方法扩增HIV-1POL区基因,进行基因型耐药性分析。结果:抗病毒治疗效果经实验室评价及分析表明,接受治疗的人群,坚持服药的病情明显好转,服药依从性对病情趋势变化具有显著影响(P<0.05);治疗后患者CD4+淋巴细胞计数显著高于未治疗的患者(P<0.05);治疗后的患者血浆病毒载量显著降低。病毒耐药谱的变化表现为,对非核苷类逆转录酶抑制剂类药物具有高度耐药性的毒株;在此基础上,陆续出现对核苷类逆转录酶抑制剂类药物具有低度和潜在耐药性的毒株1]。结论:河南省部分地区HIV耐药性毒株的出现已经达到较高水平。因此,让艾滋病患者提高服药依从性、减缓耐药性毒株的发生和流行、确保艾滋病抗病毒治疗成功具有重要的意义。

The drug resistance study of HIV anti-virus treatment of Henan province

Objective:To understand the effect of HIV anti-virus and the treatment affection of HIV drug resistance strain appearance.Methods: Choose 3 counties that have more AIDS patients as research sites,select HIV patients not only that receive anti-virus treatment but also those haven't receive treatment.Take personal interview and follow up fill in investigate form.Check the change of cd-4 lymphocyte count.Observe blood plasma virus load,laboratory evaluate anti-virus treatment effect.HIV-1 pol gene was amplified by PT-PCR,analyze it's genetic drug-resistance.Results: Results labortary evaluate and analyze of anti-virus treatment indicate HIV patients that receive reatment insist on taking medicine,their symptom much better than before.And drug compliancy have great effect to patient's condition change.(P<0.05).The CD~ -4 lymphocyte count of patients indicate that patients receive treatment much higher than patients that haven't receive treatment(P<0.05).The blood plasma virus load drop greatly after receive treatment.The virus drug resistance change shows that HIV has higher drug-resistance straina of NNRTI.On the base of this situation,in succession appear lower and potential drug resistance virus strains for NRTI medicine.Conclusion: The appearance of HIV drug-resistance strains in part of Henan province reach high level.In this situation,improve drug compliance of AIDS patients have great meaning to slow down the appear and epidemic of drug resistance strains,ensure AIDS anti-virus treatment successful.
