2所不同国家医院剖宫产围术期抗生素预防性应用比较

目的:探讨有效、合理、方便、经济的剖宫产围术期抗生素预防性用药方案。方法:2所医院170例剖宫产者采用2种不同的用药方案,其中2011年摩洛哥拉西迪亚省立医院妇产科85例设为Ⅰ组,产妇在术中断脐后静脉点滴氨苄青霉素2.0g,术后氨苄青霉素1.0g,q6h,给药4次;2011年上海市长宁区中心医院产科85例设为Ⅱ组,术前30min静脉点滴头孢唑林1.0g,术后头孢唑林1.0g,q12h,持续3d。比较2组术后发热、产褥病率、伤口愈合情况、产后恢复和预防感染的效果。结果:Ⅰ组的抗生素用药剂量少、用药时间短、术后恢复快,预防感染效果与Ⅱ组相同。结论:Ⅰ组方案优于Ⅱ组。

Comparison of Perioperative Prophylactic Application of Antibiotics for Cesarean Section between Two Hospitals from Different Countries

OBJECTIVE:To investigate effective,rational,convenient and economic program of perioperative prophylactic application of antibiotics in cesarean section.METHODS:170 cases of cesarean section in 2 hospitals were given two kinds of drug regimens,among which 85 cases of pregnant women from Morocco Rachidia Provincial Hospital as groupⅠwere given ampicillin 2.0 g i.v.gtt after cutting off the umbilical cord during operation,and given ampicillin 1.0 g i.v.gtt,q6h,4 times after operation;85 cases of pregnant women from Shanghai Changning District Central Hospital as groupⅡwere given cefazolin 1.0 g i.v.gtt 30 min before operation and cefazolin 1.0 g i.v.gtt,q12h for consecutive 3 d after operation.The postoperative fever,postoperative puerperal morbidity,postoperative wound healing,postpartum recovery and the effect of infection prevention were compared between two groups.RESULTS:GroupⅠshowed small amount of antibiotics used,short medication time and rapid postoperative recovery,and the effect of infection prevention was the same as groupⅡ.CONCLUSION:The regimen of groupⅠis better than groupⅡ.
