摘    要:目的:探讨触诊阴性的乳腺癌钼靶表现,评价核芯针微创活检的临床运用价值。方法:31例触诊阴性的乳腺癌均行常规钼靶摄片,21例采用X线立体定位系统装置、配套的自动弹射活检枪及核芯针进行微创活检。所有病例均经手术病理证实。结果:钼靶片表现为簇状钙化21例,孤立结节2例,局限性致密或结构紊乱4例,放射状毛刺影4例,21例活检病理结果导管原位癌6例,小叶原位癌1例,浸润性导管癌14例,21例行核芯针微创活检病例术后病理结果导管原位癌5例,小叶原位癌1例,浸润性导管癌15例。结论:钼靶对检出触诊阴性的乳腺癌具有重要价值,核芯针微创活检可获得准确的病理组织学结果。

Abstract:Objective:To study mammography features of non-palpable breast cancer and evaluate the clinical values of the application of micro-invasive core needle biopsy.Methods:31 cases underwent mammography. A computer assisted stereotactic mammography was used as the guidance for 21 cases nonpalpable breast cancer. Core needle biopsy was performed by a spring-loaded automatic biopsy gun with a needle. Results:Among 31 cases, various features were found in their X-ray appearances: cluster microcalcifications in 21 cases, nodular lesion in 2 cases, architecture distortion in 4 cases, spicules in 4 cases. Pathologic diagnosis of biopsy specimen was follows: ductal carcinoma in situ 6 cases, lobular carcinoma in situ 1 case, invasive ductal carcinoma 14 cases. Postoperative pathologic diagnosis was as follows: ductal carcinoma in situ 5 cases, lobular carcinoma in situ 1 case, invasive ductal carcinoma 15 cases.Conclusion:Mammography is valuable in diagnosis of non-palpable breast cancer. Micro-invasive core needle biopsy can gain exact pathologic diagnosis.
