磁共振黑血结合亮血技术对急性缺血性脑卒中双侧颈内动脉斑块的差异性研究

摘    要:目的:利用磁共振黑血结合亮血技术分析急性缺血性脑卒中患者症状侧与非症状侧颈内动脉斑块的形态学和成分差异性,为临床预测和预防急性缺血性脑卒中事件提供有价值的参考信息。方法对2014年9月—2016年8月在我院经临床及MRI确诊为单侧首发急性缺血性脑卒中患者,在发病2 d内行规范的磁共振亮血序列和黑血序列高分辨率靶扫描。利用磁共振黑血结合亮血技术,分析、评估急性缺血性脑卒中患者症状侧与非症状侧颈内动脉斑块形态学和成分的差异性。结果急性缺血性脑卒中患者症状侧与非症状侧颈内动脉斑块所致管腔最大狭窄程度无统计学差异(P>0.05),而管壁横向最大厚度、斑块纵向最大范围及斑块体积均有显著统计学差异(P<0.01)。症状侧与非症状侧颈动脉斑块钙化发生率无统计学差异,而斑块内出血及脂质坏死核具有显著差异(P<0.05)。结论磁共振黑血结合亮血技术能有效地对颈内动脉斑块的形态学和成分进行评价分析,在指导临床预防缺血性脑卒中具有重要意义。

Study on difference on acute ischemic stroke of internal carotid artery plaque of bright blood technique combined with magnetic resonance blood
Abstract:Objective To evaluate the use of magnetic resonance blood analysis combined with the bright blood technique on the symptoms of acute ischemic stroke patients and asymptomatic side carotid plaque morphology and composition,finding differences between symptomatic and asymptomatic side of carotid plaques,providing valuable information for clinical prediction and prevention of acute ischemic stroke the event. Methods from September 2014 to August 2016 in Xinfeng County People's hospital diagnosed by clinical and MRI in the patients with first acute cerebral ischemic stroke,2 days after the onset of magnetic resonance in the specification of the bright blood sequence and black blood sequence of high resolution scanning target.Using magnetic resonance black with bright blood technique,analysis, evaluation of different symptoms in patients with acute ischemic stroke and asymptomatic side carotid plaque morphology and composition. Results the symptoms of acute ischemic stroke patients and asymptomatic side There was no significant difference in carotid artery plaque caused the largest degree of stenosis lumen (P>0.05),and the maximum horizontal pipe wall thickness,the maximum longitudinal range of plaque and plaque volumes were statistically significant differences (P<0.01).Symptomatic and asymptomatic side of carotid artery plaque calcification rate is not statistically significant,and intraplaque hemorrhage and lipid necrosis with significant nuclear difference. Conclusion MRI combined with bright black blood technique can effectively on carotid plaque morphology and composition were analyzed and evaluated, and effectively predict the events of acute ischemic stroke,is of great significance in guiding the clinical prevention of ischemic stroke.
