Snodgrass术与Duckett术在尿道下裂治疗中的疗效比较

目的：总结尿道板纵切卷管尿道成形术（Snodgrass术）与横形带蒂岛状包皮皮瓣尿道成形术（Duckett术）治疗尿道下裂的疗效并对其进行比较。方法：应用Snodgrass术与Duckett术治疗80例尿道下裂患者,分成两组,每组各40例。结果：Snodgrass术一次手术成功36例,成功率为90%;手术时间80±5分钟;术后发生尿瘘3例。Duckett术一次手术成功35例,成功率为87.5%;手术时间100±5分钟;术后发生尿瘘3例,阴茎扭转2例,尿道狭窄1例。结论：Snodgrass术与Duckett术可用于阴茎头、冠状沟、阴茎体、阴茎阴囊型尿道下裂的治疗,Snodgrass术操作简单,易于推广。对于阴茎发育差,尿道板条件不好的初次手术病例,仍以选择Duckett术为宜。

Comparison of Efficacy of Hypospadias with Snodgrass Urethroplasty and Duckett Urethroplasty

Objective:To evaluate and compare the efficacy of the Snodgrass urethroplasty and the Duckett urethroplasty in hypospadias repair.Methods: 80 patients underwent surgical treatment for hypospadias.Snodgrass urethroplasty was performed in 40 cases,and Duckett urethroplasty was used in 40 patients.Results: In Snodgrass group,36 cases onestage operation,Success rate of operation is 90%,80±5 min with mean duration of surgery,3 cases with urethral fistula;In Duckett group,35 cases onestage operation,Success rate of operation is 87.5%,100±5 min with mean duration of surgery,3 cases with urethral fistula,2 case with distortion of penis,and 1 case with urethral stricture.Conclusions: Snodgrass urethroplasty and Duckett urethroplasty can be used in distal and proximal hypospadias,Snodgrass urethroplasty is simplified to recommend;Duckett urethroplasty is suitable for the primary surgery patients with poor development of penis and urethral plate.
