股骨转子间骨折的手术治疗

股骨转子间骨折指股骨颈基底至小转子水平之间的骨折,是骨科多发疾病,有20%的老年骨折为髋部骨折,其中近50%为转子间骨折。多数学者主张早期手术治疗,以便早期活动,减少并发症,降低死亡率。转子间骨折的固定方式不断发展。其治疗的方法多种多样,主要的治疗方法为外固定支架、空心钉、动力髋螺钉(DHS)内固定、动力髁螺钉(DCS)内固定、经皮微创加压钢板(PCCP)及解剖型锁定钢板(PFP),Gamma钉内固定和股骨近端髓内钉(PFN)及其最近的改进型、联合拉力髓内钉(INTERTAN),髋人工关节置换等方式。本文将以上方法的临床应用情况进行综述。

Surgical Treatment of Intertrochanteric Fractures

Intertrochanteric fracture refers to a fracture from the femoral neck base to small rotor level and it is one of multiple orthopedic diseases.20% of senile fractures are hip fractures,nearly 50% of which are intertrochanteric fractures.Most scholars advocate early surgical treatments for early activitivities,with less complications and mortalitities.The ways of intertrochanteric fracture fixation are developing.Therapeutic methods are various.The main methods are external fixation,hollow nails,dynamic hip screw(DHS),dynamic condylar screw(DCS),percutaneous compression plate(PCCP),anatomical type locking steel plate(PFP),Gamma nail and proximal femoral nail(PFN) and its recent improved types,trigen intertan hip fracture nailing system and hip joint replacement.This article reviewed the clinical applications of the above-mentioned methods.
